I am going with the Jimmy did actaully get to go to heaven and Cas is now the only one residing there.
Errrr... not to make it even MORE depressing, but I'm not wholly convinced there is a Heaven to go to in the mythology of the show. Maybe for Jimmy because of his rainbow angelic connection, but as we've seen with Tessa time and again, there seems to be nothing after a soul is reaped (she tells Dean that all of that "going to a better place" stuff people say is a way to numb themselves to the truth/fear of nothing... or something along those lines).
It's something that puzzled me, especially in "Death Takes a Holiday", the weird way in which the show juxtaposes a known Heaven/Hell plane with a nothingness... since the former is in place to refute the possibility of the latter in religion. Thus, I came to a very long and complicated conclusion (that I'd link to, save my lj is locked) about Heaven/Hell/Ghostly plane and nothing. Namely, that Ruby's knife and the Colt are two known weapons that kill demons as opposed to sending them back to Hell (they emphasized this difference blatantly in the show, so it must mean something) and that the only time we know of humans passing over into Hell instead of being reaped to nothing is when they've made a deal (Dean, John, Ruby, Bela, etc)... and angels who die seem to really die (and not just go back to Heaven... otherwise, what would be the big deal, right?).
So, I'd say Heaven/Hell/ghostly plane all exist in a realm beyond ours, and unless you have a special ticket (deal with a demon/become a vengeful spirit/etc.) you skip on over to nothing when you're reaped. So... how does one get to Heaven? Again, Jimmy might have a special in akin to the demon trades, so it's a possibility. But for the rest of humanity... it looks pretty grim.
